Rehab at Home
Rehab at Home is an innovative rehabilitation program for persons recovering from orthopedic injuries, as well as hip, knee, or shoulder joint replacement.
- Program Expert: A Physical Therapist will be your single point of contact and will work with your practice to develop customized protocols.
- Next-Day Service: Therapy services available the day after patient is discharged, including weekends.
- Quality Patient Care:
- Therapy Frequency: High frequency therapy (5x in first week) is available.

- Communication:
- Patient Information Packet: Mailed to the patient two weeks prior to surgery, explaining the Rehab at Home Program with educational brochures.
- Pre-surgery Phone Call: A Program Liaison addresses questions and conducts a home safety evaluation via phone to ensure the home environment is optimal for patient’s post-operative return home.
